What is Smart Filler? How is it applied?

You may have heard of Ellanse, a filler containing Policaprolactone (PCL), commonly known as Smart Fillers. Smart Fillers are one of the most preferred anti-aging treatment methods recently. Smart Fillers, an injectable filler, have significant differences from hyaluronic acid-containing fillers.   While both are excellent treatments, the best choice will depend on your needs. 

I will explain these differences as much as I can about Ellansé so that you can decide if it is right for you.

What is Ellansé Smart Filler and How Does It Work?

Ellansé is a hyaluronic acid-free filler. It is injected into the skin to gently lift, shape and restore the face while replacing volume lost due to skin  Its key ingredient is Poly-Caprolactone (PCL) microspheres, a soft bioabsorbable medical polymer. PCL is suspended in a gel carrier made of carboxymethylcellulose (CMC).

When injected into the skin , Ellansé reverses skin ageing in a 2-stage process:

CMC gel gives instant lifting and contouring results. However, the effect of the gel only lasts for about 12 weeks.

At the same time, PCL microspheres begin to stimulate your skin to produce collagen. Collagen stimulation peaks at 12 weeks to give the final result.

Unlike hyaluronic acid fillers that lift due to their gel properties, Ellanse works by stimulating your body to produce collagen. Results last 2 – 4 years.

Why Ellansé   Lasts Longer Than Other Dermal Fillers?

Ellansé is made of microspheres that degrade differently from other fillers. When Ellansé enters your body, it goes through a process of “absorption” in which the molecules are gradually dissolved. The microspheres retain their original volume until almost all molecules are dissolved.

Longevity depends on the number of molecules that make up the spheres. The more molecules there are, the longer the result lasts.

Ellansé is surprisingly versatile. Ellansé can be used to treat most areas that hyaluronic acid (HA) fillers can treat:

I think both Ellansé and other Hyaluronic Acid (HA) fillers give excellent results.

Advantages of Ellansé over HA fillers:

Ellansé does not cause delayed hypersensitivity reactions (granuloma formation or hardened lump formation) that can rarely occur with HA fillers.

Ellansé does not migrate because it works with your own collagen production. (HA fillers can migrate in rare cases )

Ellansé retains its volume better over time.

The advantages of HA fillers over Ellansé include :

HA fillers have an antidote – they can be dissolved when a problem arises. Thus, both minor (such as tiny nodules) and major complications (e.g. necrosis) can be treated more easily.

HA fillers give the skin a more moisturised appearance. Therefore, they are also used as Skinboosters to improve skin texture.

HA fillers are better for fine-tuning in very sensitive areas such as the under-eye area.

HA fillers are cheaper.

If you suddenly decide to have surgery (e.g. before nose surgery ) HA fillers can be dissolved. Ellansé cannot be dissolved and you will have to wait until it is absorbed in a few years.

What are the Possible Side Effects of Ellansé Smart Filler?

Ellansé is very safe and most of the side effects are related to the injection – slight swelling, bruising and pain. These are temporary and will be resolved without any problems.

The serious side effects of Ellansé are similar to other injectable fillers – necrosis (death of the skin) and blindness. These are extremely rare and can be minimised with good medical technique.

Since Ellansé is insoluble, any overfill cannot be easily removed – so it is even more important to find a doctor you trust and with enough experience to have the procedure done for you.

How do I know if I am suitable for Ellansé Smart Filler?

As you can see, Smart Fillers are different from other fillers in many ways. Deciding on the best filler for you, depends on several factors , which are detailed above and which you should discuss in depth with your doctor.

Ellansé is also more expensive than other hyaluronic acid fillers, but in the long run, you will enjoy cost savings due to its long service life.

The outcome of your treatment will depend on several factors, including your doctor’s experience, aesthetic taste, your skin texture, your level of treatment compliance and, of course, your budget. 

After all these are evaluated, your doctor can recommend the most suitable filler for you and you can achieve the best results.
